1To be eligible for this benefit, recipient must be an active member of Health Plan of Nevada’s Medicaid plan. HPN will verify visits, vaccines and screenings before we mail the member reward. Member reward takes six to eight weeks for processing. Program is subject to change. We will mail member reward to the address on the form. Members are responsible for lost or stolen cards.

Child vaccines:

Children should complete the vaccine series on or before second birthday to receive award:

  • ✓ 4 Shots for Diphtheria (DTaP)
  • ✓ 4 Shots for Pneumococcal (PCV)
  • ✓ 3 Shots for Polio (IPV)
  • ✓ 3 Shots of Haemophilus Influenzae Type B (HiB)
  • ✓ 3 Shots for Hepatitis B (Hep B)
  • ✓ 2 or 3 Shots for Rotavirus (RV)
  • ✓ 2 Shots for Influenza (flu)
  • ✓ 1 Shot for Hepatitis A (Hep A)
  • ✓ 1 Shot for Chickenpox (VZV)
  • ✓ 1 Shot for Measles, Mumps, and Rubella (MMR)

Well-Child Visits (ages 3-11)

Children should have at least one well-visit with their doctor every year to receive reward.

Adolescent/Teen Vaccine

Teens must receive one meningococcal on or between the 11th/13th birthday, one Tdap on or between the 10th/13th birthday, and two HPV vaccinations on or between 9th/13th birthday to receive award.

Well-Child Visit (ages 12-21)

Well-child visits should be completed annually to monitor your child’s physical and emotional development.

Breast Cancer Screening

Women 50 – 74 years of age should have an annual mammogram to screen for breast cancer to receive award.

Diabetes HbA1c Testing

Adults 18 to 75 years of age with diabetes (type 1 and 2) should have an annual hemoglobin A1c (HbA1c) test to receive award.

Diabetes Eye Exam

Adults 18 to 75 years of age with diabetes (type 1 and 2) should have an annual retinal eye exam to receive award.

Prenatal Visit

Pregnant women should complete a prenatal care visit in their first trimester, on or before the enrollment start date or within 42 days of their enrollment date to receive award.

Postpartum Visit

Women should complete their postpartum doctor visit within 7 to 84 days after giving birth to receive award.
