Clinical practice guidelines
UnitedHealthcare and network providers review clinical guidelines yearly. The following
guidelines were adopted on September 22, 2025. Additionally, we implemented new prior authorization guidelines for DME effective October 1, 2025. Please read them carefully to determine if they impact your practice.
As a contracted provider, we value your input. Please review the following clinical practice guidelines and send feedback to your HPN/SHL provider advocate by December 12, 2025. If we don’t hear from you by then, we'll consider the guidelines approved without changes.
