Dental Benefits for Nevada Medicaid Members

LIBERTY Dental Plan provides dental services for Nevada Medicaid members.

Nevada Medicaid members who meet the following criteria qualify for LIBERTY’s dental benefits:

  • Children: newborns through age 20.
  • Children under age 19 who qualify for Nevada Check Up.
  • Pregnant women age 21 or older.
  • Adults with special needs age 21 or older.

LIBERTY only provides emergency dental services and some additional basic dental benefits to members 21 and over, including:

  • Periodic exams once every 12 rolling months
  • Teeth cleaning (prophylaxis) once every 12 rolling months
  • 8 Periapical (Single) X-rays and 4 Bitewing X-rays once every 12 rolling months)

Vision Care for UHC HPN Medicaid members

You do not need a referral from your primary care doctor for vision care. Simply search for a provider online and make an appointment.

Your health plan covers:

  • Exams and glasses every 12 months
  • Repairs and replacement for damaged or lost glasses
  • Glass or plastic lenses
  • Lightweight lenses if needed to balance the weight of the glasses
  • Bifocals and trifocals if necessary
  • Contact lenses for treatment of a medical condition
