Sign in to the online member center
Sign in to the online member center and select the Change Primary Care Physician button. Enter the book code for your preferred doctor, effective date and reason for the change. Then select Submit.
Be sure to enter the doctor’s entire code including the dash (-). If you don’t know his/her code, select the Check Book Code button.
You can also use our concierge, or personalized, in-person services.
In-person service is available Monday – Friday, 10:00 a.m. – 3:00 p.m., by appointment only. Please call us to schedule an appointment and a member of our team would be happy to assist you.
