LIBERTY Dental Plan provides dental services for Nevada Medicaid members.

Health Plan of Nevada’s Medicaid Plan members age 21 or older with certain heart conditions and diabetes may be eligible for our value-added adult dental program. Members who qualify2 will receive a letter or phone call with more information.
Qualified members may also get a call from one of our case managers inviting them to participate in our Dental Case Management Program.

You do not need a referral from your primary care doctor for vision care. Simply search for a provider online and make an appointment.
