If you had Health Plan of Nevada Medicaid or Nevada Check Up in 2021, you should receive a notice about renewing your coverage. To keep your coverage, you must act when or before you receive a letter from the state.
Complete the redetermination form mailed to you by the State. Then, mail the form to:

Call the Nevada Division of Welfare and Supportive Services toll-free at 1-800-992-0900. Representatives are available Monday - Friday from 8 a.m. – 4:30 p.m. They can assist you in any language.
