Health Needs Survey

The Health Needs Survey helps your health plan learn about you and what care or extra support you might need. Most people take this survey when they first join the health plan. However, we want to learn about your health each year. Your needs may change. That’s why we ask every member to do the survey once a year.

We will contact you regularly to remind you to do the survey. After you complete it, sign in to the online member center and choose “Opt In” under Healthy Food Benefit to get a $25 healthy food benefit.

We use your answers to connect you with care teams. They can help with medical needs, mental health support, and community resources.
